How to Install the 665-AV2 Floor Box
This guide provides step-by-step instructions for installing the 665-AV2 floor box, including preparation before concrete pour and activation after the pour.

Tools needed:
Screwdriver, Wire strippers
Supplies needed:
Ground screws, Leveling screws, Conduit connectors, Device panels, #6-32 screws, #8-32 screws
- Prepare the floor box
Remove the top of the box by loosening the four top screws and rotating the top plate away from the screws.
- Install grounding and conduit components
Install the four ground screws and any necessary conduit connectors as per local electrical codes.
- Set up for concrete pour
Install the four leveling screws and adjust them so that the top edge of the box is 1/16” below the screed line. Secure the box to the form and ensure it is level.
- Activate after concrete pour
Remove concrete from the access frame, break off the plastic retainers, and use the knockout to remove the plate.
- Install devices
Install devices in accordance with the National Electrical Code (N.E.C) and local electrical codes.
- Place non-metallic tunnel
Place the non-metallic tunnel in the box, removing selected tabs for feed-through, and use the snap-in partition to isolate low power service.
- Install device panels
Locate the bottom edge of the device panels in the locking tabs on the non-metallic tunnel and secure the top with the provided #6-32 screws.
- Complete installation
Finish the activation by installing the 665-CST cover using the four #8-32 screws provided with each cover.
