This guide provides step-by-step instructions for installing the 664-SC floor box, including mounting, wiring, and activation procedures.

Screwdriver, Wire strippers, Level
Supplies needed:
Leveling screws, Conduit, Concrete cover, Device panels
- Install leveling screws
Install the leveling screws supplied with the box and place the box on a firm support.
- Attach conduit
Attach conduit in the appropriate knockout (K.O.) or tapped holes as required to meet your wiring needs. Ensure that device panels are installed prior to using the center K.O.'s.
- Level and secure the box
Level the floor box to the desired height using the leveling screws and secure it to the form or ground.
- Prepare for concrete pour
Ensure the temporary concrete cover is securely in place prior to the concrete pour.
- Activate the box
After the concrete has cured, locate the box and remove any concrete covering cap. Remove the concrete cover to expose the inside of the box.
- Install device panels
Attach the device(s) to the selected panel(s) as per the catalog and install the panel(s) in the box.
- Complete wiring
Install all wiring and devices in accordance with the National Electric Code and all local codes.
- Attach the cover
Attach the 664-CST cover (ordered separately) to complete the installation.
